Before you start

• Have your list of medications ready before you start.
• Finish all forms in one go: progress cannot be saved.
• The medical history has the most required fields; three forms are simply read and sign.
• Already sent us your insurance information? Skip that part.
• The dental survey is optional and has almost no required fields.
• Scroll down to "Sign": use your finger on a phone or tablet, or a mouse on a computer.
• When all six forms are done, press "Submit" to finish.
