Dr. Bal is a Delta Dental Premier provider, and Creekside Dental Care bills most PPO dental plans for you, including Aetna, Anthem, Cigna, Delta Dental, Guardian, MetLife and United Healthcare, among others. Text a photo of the front and back of your insurance card to 916-723-4777 and we will check your benefits before your visit.
Plans we do not bill
Creekside Dental Care is unable to bill HMO plans or Medi-Cal / Denti-Cal.
No insurance?
You are welcome without insurance. Before any treatment you receive a written plan with every cost, so there are no surprises. Ask us about pay-over-time financing (for qualified applicants), so you can smile now and pay over time. Dental implants start from $575.
Using your benefits before they reset
Most PPO plans reset on January 1, and unused benefits do not roll over. If you have treatment left this year, text us to plan it before the end of the year.
